Most denials are not judgment calls. They are bundling edits, unit caps, deleted codes, and missed auth requirements: rules that were published months before the claim was created. A biller who could see those rules at submission time would never trip them. That is the whole product. Guard puts the rule book on the screen where the claim is, the moment it matters.

What Guard Checks

Built from current CMS files and the same payer knowledge base our denial agent works from.

NCCI bundling edits

1.7 million active procedure-to-procedure pairs from the current CMS quarter. Guard knows which pairs never pay together and which need modifier 59 or an X modifier, and tells you which line to fix.

Unit limits (MUE)

Bill 3 units of a code Medicare caps at 2 and the extra units deny automatically. Guard checks every line against the practitioner MUE file.

Add-on code rules

Add-on codes never pay without their primary procedure on the same claim. Guard knows all 258 add-on relationships and flags orphans before the payer does.

ICD-10 specificity and conflicts

Category headers that are not billable, codes that changed this fiscal year, and Excludes1 pairs that can never appear on the same claim.

Prior authorization flags

Payer-specific auth requirements by CPT code, including the auth vendor to call. Billing without an auth number on file is a near-certain CO-197.

Timely filing countdown

Guard knows each payer's filing window, including state overrides, computes the days remaining from the date of service, and tells you when a claim has to go out today.

Nothing is transmitted

The entire rule set ships inside the extension. Scanning and checking happen in your browser's memory. No server sees your codes, your patients, or your payers. There is nothing to BAA because nothing leaves the machine.

Download and extract

Grab the ZIP below and extract it somewhere permanent on your machine.

Open chrome://extensions

Type it in the address bar, then switch on Developer mode in the top right corner.

Load unpacked

Click Load unpacked and select the extracted folder. The Guard icon appears in your toolbar.

Open any claim in your billing system, click the Guard icon, and hit Scan this page.
